Click Here to Access Full Youtube page Acute Cholecystitis: From Gallstone to Inflammation In this video, we’ll break down the pathophysiology of acute cholecystitis, starting with an impacted gallstone and following what happens to the gallbladder as the disease progresses. Gallbladder Ultrasound Anatomy This video reviews a suggested scanning sequence that can be used to image the gallbladder. Additionally sonographic anatomy is reviewed. What is a HIDA Scan and How is it Performed? Ultrasound is the primary diagnostic test when evaluating the gallbladder and biliary tree. It is a convenient test that is easily performed at the bedside or through consultation with radiology. Seldom, though, it can be non-diagnostic, and when it cannot determine the diagnosis a HIDA scan may be useful. HIDA is considered to be the gold standard for diagnosing cholecystitis and can help for evaluating the biliary tree. Understanding how this scan is performed can further help to comprehend the utility of the exam. Click Here to Access Full Youtube page Ultrasound Guided Lumbar Puncture Performing a lumbar puncture can be extremely difficult when skeletal anatomy is not easily palpable. Ultrasound can be an invaluable tool to successfully identify skeletal anatomy and provide guidance for a successful lumbar puncture. "Walking the Needle In" on Lumbar Puncture As an acute care clinician it is critical to be able to perform lumbar punctures and there are many techniques to be successful. My favorite technique is to walk the needle in to the correct location using the inferior spinous process as a "guide." In this video I explain how to perform this technique which has helped me to be successful with performing lumbar punctures myself or guiding residents and students to a successful lumbar puncture.

Meet the Team Dr. Jared T. Marx, MD, AEMUS-FPD Dr. Jared Marx is a Professor of Emergency Medicine at the University of Nebraska Medical Center (UNMC) and Chief of the Division of Emergency Ultrasound. He has spent more than 15 years building POCUS programs, training physicians, and advancing point-of-care ultrasound education. After completing his Emergency Medicine residency at UNMC, Dr. Marx pursued fellowship training in Emergency Ultrasound at Palmetto Health. He then joined the faculty at the University of Kansas Medical Center (KUMC), where he developed the Emergency Medicine residency ultrasound program and established an Emergency Ultrasound Fellowship. Dr. Marx later returned to UNMC with the goal of rebuilding its Emergency Department POCUS program. What began with minimal clinical POCUS utilization has grown into a program performing more than 4,000 clinical POCUS examinations annually . He established UNMC's Advanced Emergency Medicine Ultrasonography (AEMUS) Fellowship and pursued accreditation as soon as the national accreditation pathway became available. He subsequently obtained AEMUS Focused Practice Designation (FPD) through the American Board of Emergency Medicine. In 2026, Emergency Ultrasound became a formal division within the Department of Emergency Medicine, with Dr. Marx serving as Division Chief and leading a team that includes three additional fellowship-trained ultrasound faculty. Dr. Marx was promoted to Professor of Emergency Medicine in 2021 and awarded tenure in 2026. His educational philosophy is the foundation of POCUS Geek: make ultrasound understandable, keep examinations focused on the clinical question, recognize the findings that matter, and use that information to make better decisions at the bedside. She graduated from Utah Valley University with her Bachelor of Fine Arts in Ballet and Minor in Business Management. While in university, she oversaw the development and creation of several research projects sponsored by Utah Valley University's Undergraduate Research Scholarly and Creative Activities (URSCA). Both of these projects combined her areas of study and the research was presented at a variety of conferences throughout Utah. Outside of business, Samantha's other career is as a professional ballerina and teacher with Virginia National Ballet. She has previously danced as a trainee under Avant Chamber Ballet.

Our Story My interest in point-of-care ultrasound began early in my second year of residency. I remember late one weekend night, an ambulance was bringing in a young woman who had passed out at home after an evening out dancing with friends. We expected an intoxicated patient. Instead, we walked into the room to find a critically ill, profoundly hypotensive woman. POCUS was not routinely used but I decided to place a curvilinear probe on her abdomen. Almost immediately, I saw extensive free fluid throughout the peritoneal cavity. Based on the imaging, I told her I was worried about a ruptured ectopic pregnancy. She quickly told me that couldn't be possible; she had been diagnosed with a miscarriage four weeks earlier. Suddenly , the pieces fit together. A previously empty uterus combined with extensive intraperitoneal free fluid and profound hypotension pointed to a ruptured ectopic pregnancy requiring emergent surgical intervention. We urgently contacted gynecology. I remember repeatedly telling the gynecologist, "She has a positive FAST exam." After the third or fourth time, the gynecologist finally looked at me and said, "I don't understand what you're saying." That moment taught me two lessons that have shaped my career. First, bedside ultrasound could reveal critical information within minutes and fundamentally change how I cared for a patient. Second, finding the answer wasn't enough. I needed to communicate what I was seeing in a way that other clinicians could immediately understand and trust. Those realizations have shaped my career path into what it is today. For more than 15 years, I have taught POCUS to thousands of medical students, residents, fellows, faculty, and practicing physicians. I have built and led emergency ultrasound programs at two prominent academic medical centers. Through all of that experience, I've become convinced of one thing: POCUS doesn't need to be complicated. That's why I created POCUS Geek. My goal is to take concepts that can seem complex and focus on what actually matters at the bedside: How do I get the image? What findings do I really need to know? And how do I use those findings to make better clinical decisions? POCUS Geek is built around practical, focused education for clinicians. No unnecessary complexity. The emphasis is on using goal-directed POCUS early in the evaluation to answer focused clinical questions and guide patient care. Because ultimately, POCUS isn't about making beautiful images. It's about getting the information you need to take better care of the patient in front of you. Learn the image. Understand the finding. Communicate the decision.
